TWO-HOUR DIABETES FOLLOW-UP CLASSES
*New* classes offered. For those with Diabetes that have already attended any diabetes educational program, Medicare can provide coverage for additional learning skills. Why not try a healthy cooking class, or a complete review class for glucose monitoring and nutritional carbohydrate counting, or a stress reduction class? All classes can help you better manage blood sugar levels.

WEIGHT MANAGEMENT AND HEALTH CLASS
This is a non-diet approach to weight loss and focuses on behaviors, self-awareness and how nutrition can affect your body and your mood

CONTINUOUS GLUCOSE MONITORING (CGMS)
A diagnostic/blinded study of blood sugar for a 72 hour period. Involves subcutaneous insertion of a small sensor that measures interstitial blood sugar levels every 5 minutes and results are stored in a monitor worn on the body for 72 hours. Information is downloaded and graphs of blood sugar levels are then sent to the physician for interpretation. Helps physician adjust patient’s oral diabetes medication, or insulin regiments to be more accurate and can help to recommend more accurate pump settings for patients who wear, or who are interested in wearing insulin pumps.

INDIVIDUAL MEDICAL OR NUTRITIONAL COUNSELING
A 1:1 one hour appointment with a Nutritionist and/or nurse diabetes educator. Physician referral is required for insurance coverage.

Fee: Medicare covers Nutritional counseling only if you have Diabetes or Renal/Kidney disease. For private insurance, we will do the pre-authorization to check coverage. If not covered, out of pocket fee applied.
