TAV (Teenage Volunteer) Program at Boca Raton Regional Hospital
Thank you for your interest in our Teen Volunteer Program. Our teen program
offers volunteer opportunities for high school students age 15-18 years
old. The adult program starts at 19 years of age.
The teenager must have a genuine desire to help others, an understanding
of their responsibility to adhere to all D-RMSL and BRRH policies, and
agree to one four-hour shift each week for at least six consecutive months.
As a volunteer you are a critical part of the team and reliability and
consistency are important. Teenage volunteers are only allowed four excused
absences during their six-month commitment. Two unexcused absences will
result in immediate dismissal from the program. No exceptions. Please
consider this before applying to our program.
Step One: Complete the teen application.
Step Two: Complete the Volunteer Medical Clearance Form – this form must be
signed by a medical provider:

Step Three: Complete the COVID-19 Consent, Release, and Waiver of Liability Form:

Step Four: Provide proof of COVID-19 vaccination(s)-On an annual basis, volunteers
are required to meet various health requirements in order to actively
volunteer at Baptist Health, including but not limited to annual Tuberculosis
Screening and Influenza Vaccination. Only those who are fully-vaccinated
(two doses of Pfizer or Moderna or one dose of J&J Janssen) will be
able to volunteer within Baptist Health facilities.
Step Five: Provide a copy of an ID (driver’s license, school ID, or passport).
The Volunteer Medical Clearance Form, the COVID-19 Consent, Release, and
Waiver of Liability Form, an official ID, and the proof of COVID-19 vaccination(s)

The deadline for submitting all paperwork is November 2, 2022. Once the
process has been completed and all documents have been submitted, the
prospective teen volunteer will receive an invitation to attend the upcoming
orientation on November 16, 2022 at 4:00pm. You will be asked to pay $25.00
